Scheme and Subscriber Information: Fill in your PRAN (Permanent retirement account Number), date of superannuation, and the name of the PrAO (Pay and Accounts Officer) or DTO (Drawing and Disbursing Officer) of the government office where you served. 3. Annuity Option: Choose your preferred annuity scheme from the options mentioned above. 4. Nominee Details: Fill in the nominee's name, address, relationship with the subscriber, and percentage allocation if choosing a joint life annuity scheme. 5. Bank or Post Office Payment Details: Provide your bank or post office account details where the annuity payments will be credited. 6. Bank Mandate: Fill out the bank mandate details, including bank account number, IFSC code, and MICR code, for electronic transfer of funds. 7. Signature and Date: Sign the form and mention the date of submission. It is essential to carefully fill out Form 101 GS and review all the information provided before submitting it to the NPS authority.
